Consultation Documents (Required)

Please scan and email the following documents to info@dpinolaw.com before your initial consultation.  If you have any questions, please call the office prior to your appointment.  

Dissolution of Marriage

 1.  Drivers License (or any form of identification with photo and signature)

2.  Marriage License

3.  Birth Certificate(s) of Minor Child(ren), if any.

4.  If there are marital assets (anything worth more than $1000, anything purchased during the marriage or gained value during the marriage despite how it is titled), a document that describes the asset and its value

5. If there are marital debt (despite how it is titled), a document that evidences how much money is owed and to who

6.  If there is an existing court order regarding child support, a copy of the court order

7.  If a child care expense exists, document from provider that shows the cost of service

8.  If health insurance exists, document that evidences the cost for the health insurance for the parent only and cost for adding the minor child(ren)

9.  Most recent paystub

10.  Most recent tax return

11.  Any other document, not listed above, that you believe will assist the Attorney in the consultation

12.  Consultation Fee

Paternity: Child(ren) Not Born of Marriage

 1.  Drivers License (or any form of identification with photo and signature)

2.  Birth Certificate(s) of Minor Child(ren), if any

3.  If there is an existing court order regarding child support, a copy of the court order

4.  If a child care expense exists, document from provider that shows the cost of service

5.  If health insurance exists, document that evidences the cost for the health insurance for the parent only and cost for adding the minor child(ren)

7.  Most recent tax return

8.  Any other document, not listed above, that you believe will assist the Attorney in the consultation

9.  Consultation Fee 

Child Support/Alimony Modification

 1.  Drivers License (or any form of identification with photo and signature)

2.  Birth Certificate(s) of Minor Child(ren), if any

3.  If there is an existing court order regarding child support and/or alimony, a copy of the court order

4.  If a child care expense exists, document from provider that shows the cost of service

5.  If health insurance exists, document that evidences the cost for the health insurance for the parent only and cost for adding the minor child(ren)

6.  Most recent paystub

7.  Most recent tax return

8.  Any other document, not listed above, that you believe will assist the Attorney in the consultation

9.  Consultation Fee 

Child Support/Alimony Enforcement

 1.  Drivers License (or any form of identification with photo and signature)

2.  A copy of the existing order and all settlement agreements
